To submit an nomination for consideration, please follow the following steps.

Step 1: Review the Hall of Fame bylaws for nominating an individual, coach, team, or friend of sports.

Step 2: Open the nomination application PDF form and print out form.

Step 3: Fill out form in its entirety.

Step 4: Mail forms to Quincy Blue Devils Sports Hall of Fame, 1535 State Street, Quincy, Illinois 62301 or you may fax the form to (217) 222-1108.
